6th Gr Parents - Intermediate will again host an Immunization Clinic for immunizations required for 7th Grade on Thurs, 5/9, 1:45-3:45. (MCV & Tdap) Parents must be present. (Also, bring ID & your child’s shot record; Medicaid card, if available.) Registration paperwork will be available on site.

Plainview Intermediate will host an Immunization Clinic for immunizations required for the upcoming 7th grade school year. The immunizations required for incoming 7th graders are MCV (Meningococcal) and Tdap (Tetanus-Diphtheria-Pertussis). Parents must be present for their child to participate in this opportunity. (Parents must also bring some form of ID, your child’s shot record and, if your child has Medicaid, please bring their card.) Registration paperwork will be available on site on the day of each clinic.
